runoffgroove Tonemender - Switch Question

Wired one up tonight, seems to work but for the tone stack switch.  It calls for a DPDT switch, but I read it over and over and didn't see what type - so I used an on/off/on. 
a) right switch?
b) see diagram, are 1a 1b 2a 2b right?
c) what about the middle 2 legs - ground??

It is an ON/ON DPDT, wired to the PCB as shown here:

I don't know when how or why, but I somehow got it in my head that it was a 3 position mid shift, and that created minor angst for me - thought 'off' might be a default position
