What to do with unused pins on CD4049 or CD40106???

connect them to ground or +V??

should the input be to some of them connected and the output not connected (when not used?)

Yes you just connect the unused inputs to ground or +V, it does not matter which. You can just leave the outputs unconnected.

Or you can put them to good use.
Use a few of the other stages to make a "second channel" a la the DoubleD:

If you're into solid-state switching, you can use two of the inverters in a DOD-style flip-flop:
